Use this declaration to reconcile name differences across documents (marriage, deed poll, transliteration, initials). Keep it factual, concise, and supported with clear evidence.

When to use this

  • Your passport shows a different surname after marriage/divorce than older records.
  • Some documents show initials or transliteration differences (e.g., middle name initial only).
  • You executed a formal name change (deed poll) and need to map legacy records to the current name.

Good practice

  • List every material variant exactly as it appears (spelling, accents, punctuation).
  • Attach the primary evidence that explains the change (marriage certificate, deed poll, etc.).
  • Include certified translations for non-English documents (with translator’s statement and contact).

Common pitfalls

  • Leaving out variants that appear on key evidence (e.g., English test or degree certificate).
  • Attaching photos instead of clear, combined PDFs — re-scan and label files sensibly.
  • Submitting a long narrative instead of a concise mapping; reviewers prefer tidy, factual statements.
